Introduction: Hypereosinophilia is rarely associated with B-cell lymphoblastic leukemia (B-ALL) and when presented, it relates to diagnostic delays and an ominous prognosis.

Clinical case: A 24-year-old male presents with a month-long history of anemia, associated with B symptoms, and eosinophilia. Initially managed for intermittent rib pain and during the course of treatment, autoimmune and infectious causes were ruled out. The bone marrow analysis shows signs of B-ALL. Normal cytogenetics. Receives an induction hyperCVAD therapy to remission, with adequate control of the disease. Conclusions: This case highlights the atypical and unusual symptoms associated with eosinophilic infiltration as an initial sign of B-ALL. The prognosis in these cases is poor, associated with the biology of the disease, and on many occasions, diagnostic delays.
