In this manuscript it is presented a case of a male adolescent patient with a polyostotic unusual presentation with an additional infectious process, because of that, the initial diagnosis was Osteomyelitis and the patient received antibiotics during a long term. Posteriorly, it was performed a biopsy which supported the diagnosis of Non-Langerhans Histiocytosis. This article is relevant in our context due to the diagnosis and the particularity of the osseous involvement of this disease, although the osseous manifestations are common, our patient did not present with the typical lesions, but with scapular lytic lesions instead. In addition, Erdheim Chester disease is really infrequently seen in pediatrics with less than 20 cases worldwide so far
