          Acquired hemophilia is a disorder difficult to diagnose, with scarce prevalence, biphasic age distribution and most common in females at early ages. In 40-50% of cases there is often an underlying cause. We present the case of a 79 years old patient with two months of widespread atraumatic bruising and hematuria. It was documented prolonged aPTT with normal PT, VIII factor 0.3% and IX factor 53.1%; title inhibitors against VIII factor of 48U Bethesda. He received treatment with anti-inhibitor coagulant complex associated with cyclophosphamide and prednisone. Low-grade urothelial carcinoma that was resected, after which control bleeding was achieved, and normalization of factor VIII levels and inhibitor disappearance was documented.
