Osteosarcoma is an uncommon malignant neoplasm that occurs mainly in the pediatric population. Frequently, its clinical presentation and diagnosis occurs incidentally, since it lacks specific symptoms. Multicentric osteosarcoma is a rarer presentation of the disease with an incidence of 1% to 2% of all osteosarcomas. A clinical case of a 13-year-old female patient is presented with a diagnosis of multicentric osteosarcoma (distal diaphysiometaphyseal region of the left femur and right femur), who receives treatment with neoadjuvant chemotherapy, radiotherapy and surgical resection of the primary tumor focus. Patient receives neoadjuvant chemotherapy, radiotherapy for pulmonary tumor foci and surgical resection of primary tumor focus with 100% tumor necrosis and reduction in number, size and activity on lung tumor. We concluded that the initial clinical suspicion of multiple foci is essential when diagnosing osteosarcoma as a strategy to improve the patient's approach, which can considerably improve survival with new diagnosis and treatment strategies, leading it to be considered a curable pathology.
